NTSB: Improperly fastened rail joint responsible for employee fatality

The National Transportation Safety Board (NTSB) recently announced that a loose, improperly fastened rail joint was the cause of a Union Pacific derailment that killed one employee.

The accident occurred on September 27, 2017 as employees were performing switching moves in UP’s Great Southwestern Yard in Arlington, Texas. The investigation showed that the end of a track joint showed evidence of wheel climb, which led to the derailment.
