The Chicago Transit Authority (CTA) has released the cause of a Green Line train derailment in early June that required 30 passengers to be rescued.

The CTA says maintenance workers moved a switch that left the tracks out of alignment, causing the Green Line train to skid off the rails. The workers have been suspended 10 days without pay and will serve a six-month probation period. The operator of the train also has been found at fault, but CTA would not release any details on operator error. The operator has been off duty since the incident.

The accident involving the southbound Green Line train happened the morning of June 6 on the tracks north of 46th Street in downtown Chicago.
