Problems at a switch might have caused the deadly Frecciarossa train derailment in Italy last week.

Early results of an ongoing investigation indicate a maintenance crew was working on a switch the night before the crash that killed two and injured 31. The railroad switch was left on, allowing the high-speed train to take an alternate track where there was a stationary freight vehicle. There is a remote system for the switch, but it was deactivated. Workers were supposed to manually turn the switch off following maintenance operations. Media outlets in Italy are reporting that the maintenance crew told the Frecciarosse Control Center that the switch was turned off.
